About this project
Med Track is a consumer health management app designed to help people take control of their personal healthcare. It solves the very real problem of missed appointments, forgotten medications, and the inability to share accurate health history with doctors. The app aggregates data from manual entries and connected health devices to build a comprehensive health timeline — giving users and their clinicians a clear, accurate picture of their health over time.
Key features & capabilities
Appointment Management
Add, track, and receive smart reminders for all medical appointments — GP, specialist, dental, physio, and more. Reminders are sent at 48 hours, 24 hours, and 2 hours before each appointment, with one-tap rescheduling.
Medication Reminders
Set up complex medication schedules including dosage, frequency, food requirements, and course duration. Smart reminders adapt to the user's daily routine and track adherence with streaks and compliance history.
Vitals Tracking
Manual entry and automatic sync for blood pressure, heart rate, blood glucose, weight, temperature, and oxygen saturation. Charts show trends over time and flag readings outside healthy ranges.
Health Records Storage
Securely store and organise test results, prescriptions, discharge letters, and referral documents. All documents are end-to-end encrypted and can be shared directly with healthcare providers via secure link.
Health Device Integration
Connects to Apple HealthKit and Google Health Connect, automatically importing data from compatible wearables, blood pressure cuffs, glucose monitors, and smart scales — no manual entry required.
Health Timeline
A chronological view of the user's complete health history — appointments, medications, readings, and documents — giving both the user and their doctor a clear, instant overview of their healthcare journey.
Challenges we solved
Challenge
Users were missing medications because generic alarm apps lacked medical context and were easy to dismiss.
Solution
Built context-aware reminders that include medication name, dosage, food requirements, and remaining course days — making it harder to ignore and easier to follow through. Adherence rate reached 94%.
Challenge
Health data was spread across different apps, devices, and paper records — impossible to share with doctors quickly.
Solution
Created a unified health timeline that aggregates data from HealthKit, manual entries, and scanned documents. Doctors can be given a secure, time-limited view of the patient's timeline before appointments.
Challenge
Users were concerned about storing sensitive health data on a third-party app.
Solution
Implemented full end-to-end encryption — data is encrypted on-device before being uploaded. No one, including Hexacod, can access user health data. This became a key trust differentiator in reviews.
